No appointment needed.  Individuals can come to the Burrell Center anytime
Monday-Friday 8:30am to 2pm to see if they qualify for services.

Online Telehealth Options Available

If individual has access to Zoom, initial assessment can be completed via telehealth.
Individuals can call for more information and to initiate the process.

What will happen at first visit?

Before meeting with a licensed professional, the client will meet with an Access Specialist to see if they are eligible for a full assessment by answering questions regarding mental health and substance abuse.

  • If they are eligible for assessment, the Access Specialist will collect demographic information and review required paperwork (consent, financial contract, human rights, fall risk, confidentiality, DMAS, no show policy).
  • Client may sign releases for other entities outside of BRBH to have access to their medical information here.

What to bring to first visit?

  1.  Photo ID, social security card, passport and/or birth certificate.
  2. Proof of insurance such as Medicaid, Medicare and/or private insurance (if applicable).
  • We need this for financial assessment.
  1.  Bring legal guardian if they are:
  • 17 years or younger
  • An adult with a guardian

Please note:  Guardians must bring legal paperwork that shows them as the guardian.

  1.  Any medical documents that describe conditions, illnesses, hospitalizations, and outpatient treatment.
