What will happen at first visit?
Before meeting with a licensed professional, the client will meet with an Access Specialist to see if they are eligible for a full assessment by answering questions regarding mental health and substance abuse.
- If they are eligible for assessment, the Access Specialist will collect demographic information and review required paperwork (consent, financial contract, human rights, fall risk, confidentiality, DMAS, no show policy).
- Client may sign releases for other entities outside of BRBH to have access to their medical information here.
What to bring to first visit?
- Photo ID, social security card, passport and/or birth certificate.
- Proof of insurance such as Medicaid, Medicare and/or private insurance (if applicable).
- We need this for financial assessment.
- Bring legal guardian if they are:
- 17 years or younger
- An adult with a guardian
Please note: Guardians must bring legal paperwork that shows them as the guardian.
- Any medical documents that describe conditions, illnesses, hospitalizations, and outpatient treatment.
